Reporting

Conduct reporting is the formal process of notifying a university or college’s student conduct office about a student or student organization’s alleged violation of the institution’s policies. It’s an essential part of maintaining a safe and respectful campus community.

The reports can cover a wide range of issues, including:

  • Behavioral misconduct: like disorderly conduct, property damage, theft, or physical assault.
  • Hazing: involving student organizations, athletic teams, or other groups.
  • Sexual misconduct: including sexual harassment, sexual assault, and stalking.

With regard to reports of alleged Code of Student Conduct violations, university conduct records are protected by a federal privacy law called the Family Educational Rights and Privacy Act, therefore any information regarding the outcome of a report will not be shared without the direct written consent of the student(s) involved.

Anonymous Reporting

UA now offers anonymous reporting of any student or student organization misconduct. 

Text UAct

You can text UAct a report that may include photos or videos. Your identity, including your name and phone number, will remain completely anonymous.

To begin, text 888-708-8433 (TIDE) from any SMS-enabled device. A UA staff member will respond during regular business hours and guide you through the anonymous reporting process.

Call Navex or Report Online

For anonymous reporting of allegations related to hazing or other violations, contact Navex. Call toll-free 24 hours a day, 7 days a week: 1-866-362-9476 or use the anonymous online reporting form.

The hotline is not a 911 or emergency service. Do not use the online reporting form or the toll-free number to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please call 911.

After processing the report, Navex will provide the information to designated University officials for appropriate action. Reports submitted to Navex will be handled promptly and discreetly. Retaliation of any kind will not be tolerated.

Non-Anonymous Reporting

Our incident reporting forms are for reporters who would like to be contacted about their report. Once the report is received, a university staff member will investigate the incident and determine an appropriate intervention/action plan for the student’s behavior, which may include a meeting with the student and others involved in the incident. Please know that you, and the witnesses you identify in the report, may be contacted to provide further information.
